Type 2 diabetes remission: what the evidence shows it requires

Dr Priti Nanda Sibal is a Co Founder of the Indian Association of Functional Medicine and served as its President before the current term. She holds an MBBS and her Functional Medicine training is through A4M, the American Academy of Anti Aging Medicine, in the United States.
She practises at Peak Wellness Functional Medicine Clinic. Her stated areas of expertise are gut health, bioidentical hormone replacement therapy and diabetes.
Two themes run through her writing for the association. The first is that type 2 diabetes is no longer accurately described as uniformly progressive and irreversible. Trial evidence supports remission for a meaningful proportion of patients, and the factors associated with it are specific: the magnitude of weight loss achieved, how recently the patient was diagnosed, residual beta cell function, and whether the change is maintained rather than simply achieved.
The second is hormone therapy at midlife, where she argues that a generation of women were denied an effective treatment on the strength of findings later substantially reinterpreted, and that the correction has produced its own excesses. Hormone therapy is a legitimate treatment for defined indications. It is not a treatment for ageing, and the decision depends on a woman’s own history rather than on a general rule.
